An incident refers to:


Any accident, near-miss, equipment failure, or unsafe act/condition.


It may or may not lead to injuries or property damage.


All incidents are recorded and investigated to prevent recurrence.

Spillage of chemicals


Power failure during a batch process


A person slipping on a wet floor


⚠️ Types of Incidents:


1. Near Miss – No harm occurred, but could have.



2. Accident – Harm or damage did happen.



3. Safety Incident – Related to personal injury or hazard.



4. Environmental Incident – Affects air, water, or surroundings.



5. Quality Incident – Affects product quality or compliance.
